Manage My Chapter

Grade Release

Use this form to add members to your chapter's academic grade report prepared at the end of each semester. (Submit to the Student Center, Room 2037)

Academic Information Confidentiality Statement

This form must be completed by the chapter president and chapter/faculty/staff advisor in order to receive the chapter grade reports. (Submit to the Student Center, Room 2037)

New Member Anti-Hazing Acknowledgement

This form is to be signed by all New Members in acknowledgment of Sonoma State's Anti-Hazing policy. (Submit to the Student Center, Room 2037)

Re-recognition Modules

Chapter's university recognition is good for a year. At the end of that year, each chapter President, Treasurer, and On-Campus Advisor must complete the re-recognition moduled.

Campus Resources

Request for Facilities and Services (RUFUS)

RUFUS’s are used to request room on campus.

Off Campus Event Notification Form

This form is to be used to register all off campus chapter events.

Travel Documents

CSU Driver Application

Must be completed by all drive. This includes a copy of their Drivers License and Car Insurance if they are driving a private vehicle.

Liability Form

Must be filled out by every member of the organization who is traveling. On the back of the form they must print their names whether they are 18 or older.

Trip Request Form

Provides the Student Involvement with information about your trip and how to contact you. The trip manifest lists the drivers and those who will be riding in their cars. If the driver is driving a private vehicle they must give us a copy of their Drivers License and their care insurance. The trip request form must be turned in a week before the trip.

Policies

Anti-Hazing

Hazing, or conspiracy to haze, is defined as any method of initiation or pre-initiation into a student organization or student body, whether or not the organization or body is officially recognized by an educational institution, which is likely to cause serious bodily injury to any former, current, or prospective student of any school, community college, college, university or other educational institution in this state (Penal Code 245.6), and in addition, any act likely to cause physical harm, personal degradation or disgrace resulting in physical or mental harm, to any former, current, or prospective student of any school, community college, college, university or other educational institution. The term “hazing” does not include customary athletic events or school-sanctioned events. Neither the express or implied consent of a victim of hazing, nor the lack of active participation in a particular hazing incident is a defense.  Apathy or acquiescence in the presence of hazing is not a neutral act, and is also a violation of this section.

Expansion and Recognition of New Greek Organizations 

In collaboration with chapter and council leadership, Sonoma State University has established an Expansion and Recognition process for students and organizations wishing to colonize.  At this time, Sonoma State University has determined that the campus is not currently open for expansion.  If there is interest, please contact Leo Serrato, Campus Life Advisor, Fraternity and Sorority Life to schedule a meeting to review the procedure.
